1) LEO Cork City will email you the link to the online application form.
2) On submission of your completed application, you will be contacted again with your clinic time.
The one-hour time slots are:
- 09:00-10:00
- 10:30-11:30
- 11:45-12:45
- 13:45-14:45
- 15:00-16:00
NB The time slots will be allocated on a First Come, First Served basis and are subject to prior receipt of the completed application form.
